ADULT RULES SHEETT

This is a REQUIRED form. Before ANY ADULT is allowed to  participate in one of our MAINSTAGE PRODUCTIONS this form must be filled out, dated and signed by the ADULT actor.

PHOTOGRAPHY RELEASE FORM

This form gives us permission to use any photos or videos we take of our cast members in newspaper, television, or future advertising ventures.

This form is given out the first day of rehearsals, but, many people fill out that form and need another.  Feel free to download this sheet and keep selling tickets.

This form is used to place an AD in any of the PREMIERE or BROADWAY programs.  All questions about placing the AD can be found on the form.
